To update your automatic payment plan information, download and print the form. Once you have filled out this information, fax it to attention customer service along with a copy of a voided check to 1-319-364-6502.

Bank Information Form Instructions

1. Fill in your company as it reads on your contract or lease (You must sign and date the form)
2. Fill in your bank or institution name along with the city and state
3. Fill in your checking account name
4. Fill in your bank routing number (This number can be found on your check and it is 9 digits)
